Transport Home & Settling In – Assistance with discharge, transport, and getting settled back at home Medication Support – Ensuring prescriptions are taken correctly and on time Personal Care Assistance – Support with bathing, dressing, and hygiene Mobility & Rehabilitation Support – Helping Clients move safely and regain strength Meal Preparation & Nutrition – Ensuring proper nutrition for recovery Companionship & Emotional Support – A reassuring presence during the recovery process Ongoing Monitoring & Coordination – Liaising with doctors, nurses, and therapists to ensure continued care
